Inpatient Rehabilitation Criteria

The Rehabilitation Team
Patients benefit from a program that is directed by a highly qualified Rehabilitation Team which includes:

Individualized Care Plan
New Hope Rehabilitation treats the whole person with regards to medical, physical, behavioral, intellectual, emotional, cultural and spiritual needs in order to restore them to their fullest potential. Medical and surgical consultation is arranged on an individual basis.

The therapy team designs an individualized program for each patient to help them achieve the maximum level of functional independence they need to return home or to the community and resume their daily living.

Our goal is not just to add years to life, we want to add life to years. We focus on increasing the quality of life in the following areas:

  • Communication: Speech and language pathologists help provide patients with a functional form of communication, verbal or non-verbal, and can strengthen thinking skills. They also assess and work with swallow difficulties.
  • Mobility: Patients and families are taught how to transfer in and out of chairs, beds, and cars with or without the use of adaptive equipment.
  • Social Skills: The rehabilitation team helps people return to work, find appropriate training, and help with re-entering into the community with social activities.
  • Self-care Skills: Therapists and rehabilitation nurses help patients relearn everyday skills such as bathing, dressing, grooming and health management.
  • Memory/Comprehension Skills: The rehab team helps patients develop the necessary skills for understanding directions, problem solving in daily living and safety awareness.
